Our goal is to understand how your disability impacts you so that we can review your request for accommodations. We value hearing your experiences, history, and perspective. In addition, any form of documentation or information that you can provide to our office can help us in working with you to develop an accommodation letter.
The more information you can share with us, the more it will help us in working with you.
You can meet with us before supplying this information and we can guide you on what may be needed to develop an accommodation letter.
Examples of Helpful Documentation or Information
- Educational records, such as a high school accommodation plan (IEP, 504 plan, etc.)
- Most recent within the past 3 years
- Neuropsychological, psychological and educational evaluation
- Most recent within the past 3 years
- VA Benefit Letter with Proof of Disability
- Audiology report
- Vision assessment
- Vocational assessment
- Proof of accommodations used on standardized exams (SAT, ACT, etc.)
- Accommodations from another postsecondary institution
This is not an exhaustive list and Ocean County College – Disability Services reserves the right to request additional current and comprehensive documentation of disability.
Forms
Medical Impairment or Chronic Medical Condition
The following verification form will assist the Disability Services staff in collaborating with the student to determine appropriate accommodations. Documentation serves as a foundation that legitimizes a student’s request for accommodations. This documentation requires the expertise of a physician or other medical professional with experience and expertise in the area for which accommodations are being requested. This professional must be an impartial individual who is not a family member of the student. The name and contact information for the professional must be clearly stated.
Verification of Psychiatric/Psychological Disorders
The following verification form will assist the Disability Services staff in collaborating with the student to determine appropriate accommodations. Documentation serves as a foundation that legitimizes a student’s request for accommodations. A diagnosis by a licensed mental health professional (includes licensed clinical social workers, licensed professional counselors, psychologists, psychiatrists and neurologists) is required. This professional must be an impartial individual who is not a family member of the student. The name and contact information for the professional must be clearly stated.
Other Verification Forms
The following documentation forms have been created as an alternative to traditional diagnostic reports. If a traditional diagnostic report is being submitted instead of a verification form, please consult the Documentation Guidelines form for complete details.
Documentation Guidelines
The following guidelines are provided for those students who have access to a comprehensive evaluation. Documentation must be completed by a qualified professional by either providing a report of the disability following the guidelines below or by completing a form for that disability.

COVID-19 Vaccine Medical Contraindication Form
This form must be completed by any student requesting a waiver from the college requesting mandatory COVID-19 vaccine. Please direct all questions or inquiries to accommodations@253000xa.com.

Medical Exemption from Mandatory Mask or Face Covering
This form must be completed by any student requesting a waiver from the college requirement for mandatory mask or face covering. Requests may only be on medical grounds as an individual with a disability or as regarded as having a disability under the ADA of 1990, as amended by the ADAAA of 2008. Vaccine Medical Religious or Sincerely Held Belief Exemption Request
Students who have a religious or sincerely held belief which conflicts with 365球赛平台’s and/or the School of Nursing’s requirement must complete this form to request an exemption from this requirement. Please direct all questions or inquiries to accommodations@253000xa.com.
